Your health is important to you around the clock–not just during office hours. The myMemorialCare electronic medical record, powered by Epic, includes a patient portal we call MyChart. It's a personalized Internet connection to your doctor's office. You can schedule appointments, request prescription refills, review your health history and more—online, any time!
How Do I Sign Up?
If your physician uses myMemorialCare and you wish to participate, you will be issued a myChart activation code during your office visit. This code will enable you to login and create your own user ID and password. If you were not issued an activation code, you may call your primary care clinic to get one or ask to sign up during you next office visit.Learn more about myChart.
