Follow-Up Care
The Jonathan Jaques Children’s Cancer Center (JJCCC) model of multidisciplinary patient care is continued in its comprehensive follow-up care. A specialty physician provides ongoing, coordinated care from initial diagnosis through treatment and beyond. An expert care team coordinates medical care, provides psychosocial support, and ensures the needs of patients and their families are met for the duration of their illness and throughout childhood. The staff also facilitates transition to adult care when necessary.
Hematology Oncology – Sickle Cell Center
Hemophilia & Thrombosis CCS Clinic
Designed to follow patients with bleeding disorders in a multidisciplinary setting, the goal of this clinic is to provide comprehensive care and education for patients and families. Pediatric specialty team members include: hematologist/oncologist, social worker, psychologist, physical therapist, nurse specialist, nutritionist, dentist, and medical assistant. Patients are seen one or two times per year.
Neuro-Oncology CCS Clinic
A specialized clinic designed to follow patients with brain tumors that require a multidisciplinary approach. Patients seen in the setting may be off treatment for many years or presently receiving treatment. Many patients diagnosed with a brain tumor benefit from a multidisciplinary approach since many have complications or late effects related to their tumor location and/or treatment. Pediatric specialty team members include: hematologist/oncologist, neurologist, psychologist, social worker, physical therapist, nurse specialist, speech therapist, nutritionist, dentist and medical assistant. Patients are seen one or two times per year or more frequently if indicated.
Late Effects “STAR” Program
JJCCC’s STAR (Survivors’ Treatment Assessment and Resource) Program helps patients, who are no longer undergoing treatment, understand and manage the late effects of cancer therapy following surgery, chemotherapy and radiation.
