If you have any additional questions regarding a provider, please call the provider's medical office. Our medical staff would be more than happy to answer your questions.

Once you have chosen your MemorialCare Medical Group provider, you will need to call your health plan to tell them you would like to switch to MemorialCare Medical Group. The phone number for your health plan is located on your membership identification card.
